Alternate Forms are a relatively new concept in the Pokémon games. First appearing in GS in the form of multi-form Unown but then capitalised upon in FireRed & LeafGreen, the purpose of Alternate Forms are to give certain Pokémon new traits, stats and attacks in order to make the certain Pokémon even more interesting and even unpredictable in battle. There's only one Pokémon from the third generation with alternate forms, while in the fourth generation, there are many many more.
Below are the forms and the explanation of what makes them different and how they are obtained. For the sake of organisation, there are two categories...true form changes and minor form changes.
#386 Deoxys
Deoxys is rather unique in that he has 4 seperate forms. In the 3rd generation games, these forms were spread across the different versions and whenever Deoxys was traded over, the form changed to that, recalculating the stats based upon the Base Stats of that particular Pokémon. EVs however will stay, so if you have. Due to this, you can end up getting say an ExtremeSpeed Defense Deoxys form by trading Deoxys from Emerald over to LeafGreen. In Diamond, Pearl & Platinum however, you have the ability to change Deoxys' form at will utilising the meteors in Veilstone City. To change forms in Veilstone, the stone order is this:
- Top - Speed Deoxys
- Middle - Normal Deoxys
- Bottom Left - Attack Deoxys
- Bottom Right - Defense Deoxys
- Top Left - Normal Deoxys
- Top Right- Attack Deoxys
- Bottom Left - Defense Deoxys
- Bottom Right - Speed Deoxys
Next, in the Nacrene City museum, you have the ability to change Deoxys form. Here, you don't go to specific meteorites but instead there is just one single meteorite and the forms will cycle through whenever you interact. In Ambrette Town's museum in Kalos and Hokulani Observatory in Alola, you can interact with the meteors here in order to change your Deoxys form. Just go up to it with Deoxys head of your party and your Deoxys will change form. There are no limits on how many times this can be done.
Battling with Deoxys in the 3rd generation games is rather unique. As all versions are compatible, many will believe about the issue of clashing Base Stats. However, the stats and the attacks of Deoxys will remain on their respective games so even if you're in FireRed against LeafGreen, it will be Attack Deoxys VS Defense Deoxys. However, the sprite you will see your opponent's Deoxys as will always be the Normal Form. This also happens in Colosseum, but not in XD where all the forms are seen. In the 4th generation however, as you can have any of the 4 forms and change them at will, you will see that particular form in battle.

#412 Burmy/#413 Wormadam
Burmy is a very unique Pokémon in that its form is ever changing between the three cloaks; Plant Cloak, Sand Cloak & Trash Cloak. These are changed dependant on the area that Burmy last battled upon.
If its on Grassy areas then it'll be Plant Cloak due to picking up leaves.
If in caves, beaches or sandy areas in general then it'll be Sand Cloak due to picking up the rocks and sand.
If in buildings or on roads then it'll become the Trash Cloak.
However, as a Burmy, it gets no use out of these changes as they are purely cosmetic, but evolve a female one with its last battle in one of those places, and it evolves into a Wormadam.
Wormadam is different to Burmy in that once it has evolved, it will keep the form that Burmy was in when it evolved so it will always be Plant Cloak for example. With Wormadam however, its learnset, stats and even its type are completely different for each form so you have to make sure you have the Burmy that you want when you evolve it. Luckily you can cancel evolution so if Burmy is of the wrong cloak at the start of evolution, just cancel and go to another place and evolve. All forms are interconnectable in Diamond, Pearl & Platinum and can be battled and traded with ease.
#479 Rotom
Rotom is another Pokémon that obtained new forms. In Pokémon Platinum, at certain times, you gad the ability to download the Secret Key over Wi-Fi. This Secret Key gives you access to a new room in Eterna's Galactic Building. In HeartGold/SoulSilver, you need to just take a Rotom to the elevator in Silph Co while. In these rooms, there are five different items on the floor. If you have Rotom in your party and you select one of them, it will change Rotom into one of five other forms. Alternatively, in Black & White, you just need to interact with some boxes in the warehouse in the Route 9 department store and in X & Y, you have to interact with a box in the Pokémon Laboratory in Lumiose City.. This has Rotom possess the appliance. When he possesses the appliance, his stats are boosted and it can learn a specific attack. If you change forms, Rotom will remove this attack and learn another one so they cannot learn all of the attacks possible. All forms share the same moveset and statistics so the only difference in them is the special move that they have. The forms that Rotom changes into are as follows
- Lawnmower - Cut Rotom
- Fridge - Frost Rotom
- Oven - Heat Rotom
- Fan - Spin Rotom
- Washing Machine - Wash Rotom
Even though Rotom is in all three games, the forms are exclusive to Pokémon Platinum and as such, Rotom's forms are not in Diamond & Pearl. Like other forms, when you go into the Union Room or the WiFi Club, then Rotom will revert forms from its form into the base form, lowering its stats and forgetting its signature move. However, if you go for Battling in the Local Wireless Colosseum area, then Rotom will keep its form, its stats and its signature attack. In Platinum to Platinum battles, you will see the forms all as the forms to which they have transformed. Diamond & Pearl players however will see Rotom as its base form, despite it keeping the higher stats of its form. Rotom also reverts form when you enter it into one of the Battle Frontier facilities.

#483 Dialga
Dialga was given an Origin Forme within Pokémon Legends: Arceus. This form featured a change in design with a change in the stats. It was changed by using the Adamant Crystal item.
When in Origin Forme, Dialga's Roar of Time move increases in power to 140 Power but with a reduced accuracy of 75

#484 Palkia
Palkia was given an Origin Forme within Pokémon Legends: Arceus. This form featured a change in design with a change in the stats. It was changed by using the Lustrous Globe item.
When in Origin Forme, Palkia's Spacial Rend move decreases in power to 80 Power with a reduced accuracy of 85 but a higher chance of critical hits

#487 Giratina
Giratina was the first legendary non-event Pokémon in the game that has alternate forms dependant on the game it is on as well as external factors. Giratina's Altered Forme is found on Diamond & Pearl while it's Origin Forme is found exclusively on Platinum. It is also the first Pokémon form change that has the size and mass of the Pokémon change. In the Pokémon mythos, the Origin Forme is the form that Giratina is in in it's home dimension while it is in the Altered Forme when it enters ours, and that remains true in Pokémon Platinum. However, if you attach the item, the Griseous Orb, to Giratina, then it will remain in the Origin Forme.
Giratina is catchable in both games and as such, its form will change easily when you trade between Diamond & Pearl and Platinum. However, when you go over WiFi into the WiFi Club or into the Trade Centre, the Griseous Orb will return automatically to your bag and Giratina will revert into its Altered Forme. Over local wireless however, you have the ability to battle with Giratina in its Origin Forme. PtHGSS to PtHGSS will have the sprite showing that it is under its Origin Forme so you know exactly what you're up against. If you have a battle of PtHGSS VS Diamond/Pearl, then on the Diamond & Pearl games, Giratina will appear in its Altered Forme. It will keep the stats for its Origin Forme and the boosts of the Griseous Orb, but the Diamond & Pearl players do not notice it.

#492 Shaymin
Shaymin is the 4th generation event legendary Pokémon that has an alternate form. Shaymin's Land Forme is found on Diamond, Pearl, Platinum, HeartGold, SoulSilver, Black & White while its Sky Forme is found on Platinum, HeartGold, SoulSilver, Black & White. Being an event Pokémon, you do not have the ability to capture it in-game without an added event item so you won't be seeing this one as often. Shaymin is the first Pokémon to have interchangeable forms that have different types and its form can only be changed by using the item; The Gracidea Flower. Shaymin's Sky Forme is also dependant on the time. It can only be utilised during daylight hours of 4am to 8pm. As soon as it hits 8:01pm if you are in the overworld, or as soon as you exit battle, it will automatically revert to Land Forme. It will also revert if you place it in the Box.
Like Giratina, Shaymin's forms have certain compatibility across the multiplayer bands. Over WiFi and in Trade Rooms, Shaymin will automatically revert back into its Land Forme. However, over local Wireless battles, Sky Forme Shaymin will remain in its Sky Forme for the duration of the battle. Platinum to Platinum players will easily be able to distinguish its forms. But like Giratina, if you battle Diamond/Pearl to Platinum, the Diamond & Pearl players will see the Shaymin in Land Forme regardless of if it is in Sky Forme or not. It will however keep the stats, typing & ability of Sky Forme so their strategy may not work due to the typing so in the interest of fairness, it might be best to tell them that its Sky Forme

#550 Basculin
This Pokémon has two forms, the Red Stripe Form and the Blue Stripe Form. These forms are not interchangeable and are dependant on its appearance at capture. The forms typically are version exclusive, with White-Striped Form being introduced in Legends Arceus.
In addition to the change of appearance, the forms can have different abilities. While their Hidden ability, Mold Breaker, and their second ability, Adaptibility remain constant between all forms, each form has a different potential ability.

#555 Darmanitan
Darmanitan's form change is unique in that not all Darmanitan have the ability to do it. If you have evolved a Darmanitan from Darumakka, then you will not be able to get his alternate form. To get this form, you either need to get a Darmanitan off of the Dream World, or get one of the six that are sitting outside the Ancient Castle in the Resort Desert.
These Darmanitan have the ability; Zen Mode which, when Darmanitan's Hit Points get to be less than 50% of their full value, Darmanitan will change into Zen Mode. This change is radical and changes both Darmanitan's stats and his type.

Deerling and its evolution Sawsbuck each have four different forms. These forms are dependant solely upon the season that is currently in play. As the Season is not controllable by you, you have no control over what form these Pokémon take. but if you store it in the box, it doesnt change so you have it in the seperate form. The forms have no difference in learnsets or stats but are merely an aesthetic change to show off the new mechanic of the seasons.

#646 Kyurem The forms of Kyurem are unique in that they take another Pokémon and completely envelop them in order to create the new form. This is done using the special item, DNA Link Pin which is found in Giant Chasm after capturing Kyurem. When you use this item, you have to select Kyurem and either Reshiram or Zekrom. This will merge the Pokémon together into a far stronger Pokémon. This can be reverted at any time, using the item. Any changes to Kyurem such as Level and EVs carry over to each form of Kyurem, while Reshiram or Zekrom remain untouched. For example, if you fuse a Level 70 Kyurem and a Level 70 Reshiram, and level Kyurem up to Level 92, when you defuse them, Kyurem will remain Level 92 and Reshiram will still be Level 70. However, the moves on Kyurem will reset to the last four in its level up from the current level.

#648 Meloetta
Meloetta is an event Pokémon and also has two different forms. These forms differ in both stats and type. To change the form of Meloetta, you need to take it to the café in Hiun City where you'll meet a character who will teach it a brand new move. This move, Ancient Voice, will allow you to change Meloetta's form at will, however it only changes when its used in battle.

#658 Greninja
Greninja also has a brand new form, one that ties in with the Pokémon anime. Using the demo, you can obtain a special Greninja with the Battle Bond ability. When Greninja manages to defeat an enemy in battle, the ability will activate and turn Greninja into Ash-Greninja for the rest of the match. It cannot be bred.

#681 Aegislash
Aegislash's forms are the first set of forms to actually showcase the concept in Kalos. This Pokémon will change its form depending upon what moves it makes. The Formes are unique in that they completely swap their base stats with a focus on Attack in Blade Forme and Defence in Shield Forme If it makes any attacking moves, it switches to Blade Forme, whereas if uses the move King's Shield, it will be in its Shield Forme. Whenever it is sent out into battle, it is in its Shield Forme allowing for switching in without damage.

#710 Pumpkaboo & #711 Gourgeist
These two Pokémon also have got a unique form difference. However, unlike other form changes, these forms cannot be changed after capture. When you encounter the Pumpkaboo in the wilrd, it will be one of four possible sizes: Small, Average, Large and Super Size, with Super Size being the rarest. The difference is actually in their stats. While as a Pumpkaboo, its Hit Points go up, the larger it is, but the Speed decreases wheres when it has been traded to evolve into a Gourgeist, its Attack stat also starts getting larger for the larger forms while Speed decreases.
#718 Zygarde
Zygarde has a rather unique form change. First, you need to collect Zygarde Cells & Cores from around the Alola Region. When you have collected 10 or 50 of them, go to the building in Route 16 where you can assemble a Zygarde. You'll assemble them based on the amount of cells you have collected. This is where you need to change the form.
If you have collected all 100 Cells & Cores, you can then create a special Zygarde with the Power Construct ability. This Zygarde cannot be separated and has its form changed between 10 and 50% Formes using the Zygarde Cube item. When these form's health drops below 50%, they will change form into the powerful Complete Forme for the duration of the battle..
#720 Hoopa
Hoopa's forms are another unique set. Hoopa's second form is only available in Pokémon Omega Ruby & Alpha Sapphire and represents a big change. When you use the Prison Bottle item on Hoopa, its form will change between the Confined and Unbound forms, causing a change in type, stats and even movepool. Hoopa Unbound's signature move Hyperspace Fury can only be used by Hoopa Unbound. If Hoopa Confined tries to use it, it will fail. Hoopa Unbound will also revert to Hoopa Confined when you put it in a box and battle box. This form change is also the first ever where the alternate form has got a different category, in addition to height & weight changes
#741 Oricorio
Oricorio's form change is a rather simple one. Its form is mostly just dependant upon which island of the Alola Region it is caught on. This affects its type, as well as the type of its move Revelation Dance. However, these forms can be changed by using the special Nectar items that are found in each island. Simply give Oricorio the nectar and its form will change.

#745 Lycanroc
Lycanroc's form change is a rather unique one in that it's not only determined by evolution, it's determined by evolving in a specific game. If you're playing on Sun, it'll be in the Midday Form and if you're playing on Moon, it'll be in the Midnight Form. Each form has a different learnset, stat spread and abilities. The other one can be caught in the game.
In Ultra Sun & Ultra Moon, you can get a special Rockruff with Own Tempo that can evolve into Dusk Form
#746 Wishiwashi
Wishiwashi is one of the more unique Pokémon in Sun & Moon. This Pokémon as standard has the lowest Base Stat total in the game but its ability, Schooling allows for it to change form. When it is over Level 20 and has over 25% of health, it will change form into its massive and powerful School Form. If its health drops below 25%, it changes form back to Solo Form. If its health goes back to over 25% then it will go back to School Form

#774 Minior
Minior is another Pokémon with a form change based on Hit Points. When sent into battle, Minior will bring its shell up over itself which protects it from status conditions, but when its Hit Points drop below 50%, its Shields Down ability is activated and it switches to its Core Form. This switches the Defense and Attack base stats, and also doubles its Speed. Its capture rate while in Core form is also much higher than when it is protected

#800 Necrozma
Necrozma also got some new forms within this game. These forms are fusions with the Pokémon Solgaleo & Lunala. These retain the types of those Pokémon but keep Necrozma's Prism Armor ability. To change form, you need to use the N-Solarizer and N-Lunarizer items respectively. These will fuse Necrozma with Solgaleo & Lunala respectively. You can only have one of each form in your game.

Necrozma also got one final form in these games. This is a form obtained through an Ultra Burst, this is a feature akin to Mega Evolution and Primal Reversion. If you have the Ultra Necrozium Z attached to a Dusk Mane Necrozma or a Dawn Wings Necrozma, an Ultra Burst button will appear. This will allow you to change form to Ultra Necrozma. Like Mega Evolution, all stats including Speed are counted straight away. In the following turn, it can also use the Z-Move Light That Burns The Sky

Toxtricity has two special forms, Amped Form and Low Key Form. While both can be obtained in raids at any nature, when you evolve Toxel into Toxtricity, the nature of Toxel determines which form you get. They have slightly different movepools

Eiscue is a new Pokémon with an ability that protects it. When sent into battle, it will be in its Ice Face, but if it gets damaged by an attack, the ice on its face breaks and it shows its other form, the Noice Face. It can revert when Hail is in effect. These forms have different stats

Zacian is one of the Legendary heroes of the past. Due to that, its form had changed quite considerably over time and its stats had been lowered. However, if you attach the Rusted Sword item to it, it will change its form which includes a new type and a stat boost.

Urshifu is a Legendary Pokémon found on the Isle of Armor. When you have bonded with it, you can take it to the Towers in order for it to evolve. The Tower you take it to determines the form. The forms have different movesets, stats, type and Gigantamax Form.

Calyrex is a new Legendary Pokémon which features a new form that you obtain in the course of the story. Like Kyurem and Necrozma, this form change is actually a fusion between Calyrex and another Pokémon. In the story, you actually capture Calyrex in this form.
You can change forms by fusing using the Reins of Unity item. You need both Calyrex and the relevant Pokémon to change the form and you need a spare slot in your team to unfuse them.

#201 Unown
Unown was the first Pokémon in the games to have different forms. Unlike the aforementioned forms on this page however, Unown's forms are all purely cosmetic, despite fitting into the mythos made for Unown during GSC. There are 28 forms of Unown, each representing one letter of the alphabet and, as of the 3rd generation, 2 of them featuring punctuation marks. In Gold, Silver & Crystal there was an Unown Dex plugin to the Pokédex that allowed you to keep track of each forms. This however, has not returned. Below is a table showing all of Unown's forms.
#351 Castform
Castform is the first Pokémon to have interchangeable forms based entirely upon attacks that it uses. With the introduction of weather attacks, a Pokémon was eventually devised that makes use of these attacks and thus further benefit it and thus Castform was created. Using the three main Weather attacks; Sunny Day, Rain Dance & Hail, Castform's visual appearance and type changes and its signature attack Weather Ball changes with these changes to make Castform much more unpredictable.

#421 Cherrim
Cherrim is another Pokémon who's form changes when a weather attack is used. Utilising its ability, Flower Gift, if Sunny Day or a Pokémon with Drought has been used, Cherrim changes to a blooming blossom. While this does make Cherrim more susceptable to Fire-type attacks, Cherrim's Attack and Special Attack is raised. Once the sunlight ends however, Cherrim reverts back to its closed form.

#422 Shellos/#423 Gastrodon
Shellos and Gastrodon are rather unique in that while they both have two forms each, the forms are not interchangeable and they make absolutely no difference to the Pokémon. The two forms of Shellos & Gastrodon indicate what part of Sinnoh that they were originally captured in. If they're caught on the West side of Sinnoh (to the west of Mt. Coronet), then they will be the pink West Sea forms, whereas if they are captured on the east side of Sinnoh. As these forms are not interchangeable, you're stuck with them, but in the routes where they reside, they are rather abundant Pokémon so you can easily get both if you do so wish. It is currently unknown if Shellos & Gastrodon that are hatched or obtained in other regions will match these conventions or be completely new.

#493 Arceus
Arceus, the astrongest legendary Pokémon that sits at the end of the Pokédex (as of the 4th generation), is rather unique in that its type and appearance will change if its holding one of 16 special items. These items, when on other Pokémon, boost certain types. However, when they are attached to Arceus, they have that effect but also change its type and form to match that plate. There is one plate for each type which means 17 different Arceus forms. Arceus' signature move Judgment also is affected by these changes. However, due to these forms being dependant on both Arceus' ability and the item, its easy for an opponent to disable it with certain attacks and abilities. Vivillon's Form differences are unique in that you cannot change them or even natively get the different forms in your game. Instead, you get the version based on your geographic location set on the Nintendo 3DS when the Pokémon is caught or hatched.

Flabébé's form change is something merely aesthetic rather than a functional form. In Pokémon X & Y, you will find and capture numerous Flabébé, each with different coloured flowers depending upon the location, typically based upon the flower bed you capture it in, but with Orange and White ones being rare in any. These forms are not interchangeable and do not have an effect on the stats of the Pokémon. This form then carries on through its evolution so if you have a red Flabébé and evolve it, you will get a red Florges

Furfrou is another aesthetic form change, however it is one which you have complete control over. When you have a Furfrou, you will be able to groom it. This is done at Frieser Furfrou in Vernal Avenue in Lumiose City and costs 500P per trim. If you ignore its grooming regime, its hair will grow back to the size of it when it is captured after five days. It will also revert if you put it in a box.

Silvally is a unique Pokémon that was created to defeat the Ultra Beasts. As it is based on Arceus, it has the unique RKS System ability which changes its type, as well as the type of its move Multi-Attack. This is done by attaching the unique Memory items.

Cramorant's forms are a change in battles. When it uses the moves Surf or Dive, your Cramorant picks up its prey and changes form. When it's hit, it shoots out that prey at an enemy to cause damage. If it has over 50% Hit Points when it changes form, it'll change into Gulping Form and if it has 50% or less then it'll change into Gorging Form. The use of Gulp Missile from Gulping Form will do damage and lower opponent's Defense. The use of Gulp Missile from Gorging Form will do damage and paralyse the opponent.

Sinistea and Polteageist are rather unique in that they have alternate forms that many can play the game without noticing. Sinistea and Polteageists are ghosts that inhabit a cup and a teapot. These cups & teapots are said to be rare in Galar, but there are lots of forgeries. With this, it shows Sinistea & Polteageist having unique forms, each with their own Pokédex entry. The non-forgery Sinistea appears 1% of the time in theThese Pokémon can't actually be determined physically but only by how they evolve. The forgeries use the Cracked Pot to evolve but the very rare ones require the Chipped Pot to evolve
There is one way to tell which form you have by appearance, however. For a split frame while it's animating in the Pokédex or when you Dynamax Polteageist, the non-forgery form has got an authenticity stamp.

Alremie has a vast number of forms, numbering in over 60. These forms are obtained when you evolve its pre-evolution, Milcery. To get them, attach a Berry Sweet, Clover Sweet, Flower Sweet, Love Sweet, Ribbon Sweet, Star Sweet, Strawberry Sweet to the Milcery and you have to spin the control stick a certain way. Morpeko is a Pokémon with an aesthetic form change. While Morpeko has the ability Hunger Switch, when the turn ends it will change to its other form. This form change doesn't do much but also affects its Aura Wheel move making it Electric-type in the Full Belly Mode and Dark-type in Hangry Mode.
